Position: Case Manager

Program: Family Wellness & Empowerment Program Reporting to: Director of Family Services Status: Full-time, Permanent

Position Summary

The Case Manager provides trauma-informed, client-centered case management to women and children fleeing family violence within Wings’ Second-Stage Shelter program. This role supports families in stabilizing, healing from trauma, and building independence through structured goal planning, risk assessment, and coordinated service delivery.

Case Managers play a key role in advancing the Family Wellness & Empowerment Program by supporting long-term outcomes related to safety, housing stability, and self-sufficiency.

Key Responsibilities

Case Management & Client Support

• Develop and maintain supportive, professional relationships with women and children using a trauma-informed, strengths-based approach

• Conduct intake, comprehensive psychosocial assessments, and ongoing needs assessments

• Co-develop individualized service plans using the Wings Self-Sufficiency Matrix

• Monitor progress, adjust plans as needed, and evaluate outcomes through regular case reviews

• Support families in achieving goals related to housing stability, parenting, health, education, and economic independence

Safety Planning & Risk Assessment

• Conduct risk assessments and develop individualized safety plans

• Provide crisis intervention and ongoing emotional support

• Support clients in navigating legal systems (e.g., protection orders, custody, court processes)

• Maintain awareness of family violence dynamics and respond appropriately to risk and safety concerns

Advocacy & System Navigation

• Advocate for clients within systems including housing, legal, financial, healthcare, and child intervention

• Support access to income supports, childcare, employment, and permanent housing

• Coordinate referrals and ensure continuity of care through warm hand-offs to community partners

Program Delivery & Group Facilitation

• Work with the Community Learning & Empowerment Director (CLE) on a monthly basis to plan and facilitate supplementary programming aligned with the Family Wellness & Empowerment Program (e.g., life skills, parenting, trauma recovery, empowerment)

• Encourage client participation in structured programming

• Contribute to a culturally responsive, inclusive, and empowering program environment

Family & Child Support

• Support healthy parent-child relationships and positive parenting practices

• Promote child safety, development, and emotional well-being

• Collaborate with internal programs (e.g., childcare, health services) to support whole-family outcomes

Documentation & Reporting

• Maintain accurate, timely, and confidential client records

• Complete case notes, assessments, reports, and program documentation in alignment with organizational standards and funder requirements

• Track outcomes and contribute to program evaluation and reporting

Collaboration & Professional Practice

• Participate in team meetings, case conferencing, and coordinated care planning

• Build and maintain effective working relationships with community partners

• Represent Wings professionally and uphold organizational values and standards

• Contribute to program development and continuous improvement initiatives

Qualifications, Experience & Knowledge

• Diploma or degree in Human Services, Social Work, Community Support, or related field preferred.  

• 3–5 years experience in family violence services, shelter operations, transitional housing, or related environments.

• Registration with a professional body (where applicable) is considered an asset

• Experience providing case management to individuals and families impacted by family violence or gender-based violence

• Strong knowledge of trauma-informed care, strengths-based practice, and cultural safety

• Experience in housing, shelter, or community-based support programs is an asset

• Knowledge of community systems including housing, legal, health, and income supports

Core Competencies

• Strong assessment, case planning, and problem-solving skills

• Effective crisis intervention and risk assessment abilities

• Excellent interpersonal, communication, and relationship-building skills

• Ability to work independently and collaboratively within a team

• Strong organizational and time management skills with the ability to manage a full caseload

• Group facilitation skills and experience

• Proficiency with documentation and computer systems

• Class 5 license and access to reliable vehicle with minimum $2M Liability Insurance
